Laminectomy: without the posterior elements intact, flexed sitting can increase reliance on remaining stabilizing structures and surrounding musculature earlier than desired.
Fusion: early flexion under sustained load can place strain on instrumentation and the developing fusion mass before osseous union is established.

Clinical Note

Patients frequently report that pain is worse after watching television or riding in the car, both of which involve prolonged flexed sitting, rather than after standing activities. This pattern is often a posture finding rather than a sign of new pathology, and it is worth asking about directly during follow up visits.

Why Standard Discharge Instructions Often Miss This

Most spine precaution handouts address movement based restrictions: log rolling, hip hinge lifting technique, and avoidance of twisting. Static posture, particularly seated posture during rest and recovery, is less commonly addressed in detail. Patients are often encouraged to rest, which they interpret as permission to spend long stretches in a recliner or on a couch, precisely the environments most likely to produce unsupported lumbar flexion.
